-Insulin

Most insulin is eliminated from body by kidneys. For diabetics with decreased GFR, you should observe your blood sugar level closely when you use insulin in case of low blood sugar level.

-Pioglitazone

The metabolism of pioglitazone is mainly by liver. If you have liver dysfunction with Diabetic Nephropathy, you should adjust its dosage.
